How we get the personal information and why we have it
Most of the personal information we process is provided to us directly by you while booking or during your appointment. We use the information that you have given us in order to record details of your assessment and therapist recommendations. These are used to guide and administer the care that you receive. They ensure that:
· The professionals involved in your care have accurate and up-to-date information on your needs and future care requirements
· There is a good basis for assessing the quality of care that you receive
· We can work effectively as a multi-disciplinary team
· Any concerns can be properly investigated, should you need to complain
Your information will only be used for professional purposes, and may be used anonymously (by removing any details that could identify you) to help us:
· Review and monitor the overall quality of care we provide, to make sure it is of the highest standard
· Make sure that our services are meeting patients needs
· Train and educate our staff
In the usual course of our business we may disclose your personal data (to the extent necessary) to third party organisation that we use to support the delivery of our services. This may include the following:
· Business partner, suppliers and sub-contractors who play a part in our contract with you
· Organisations providing IT systems support and hosting in relation to the IT systems on which your information is stored.
· Third party service providers for the purposes of storage of information eg exercise program software.
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information is your consent. You are able to remove your consent at any time. You can do this by contacting us on the above contact details.
How we store your personal information
We will take all reasonable steps to protect the confidentiality and security of your personal information, and to keep personal information accurate and up-to-date. We will take all reasonable steps to ensure data is only retained for the duration of the purpose for which it was obtained. When requested, Northern Therapy will delete personal data from current operational systems where it is no longer required. Upon request, Northen Therapy can rectify records if there is a valid reason and if this does not go against the HCPC standards of proficiency. Northern Therapy may be required to hold some details about you after your treatment with us has ended, for example for legal audit and regulatory purposes relating to healthcare.

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
